University Students
Go to Upload Your Photo to submit your photo online. Once your photo is approved, you can provision your Mobile DukeCard. If you require a physical card for lab access or medical center access, please partner with your lab manager to complete a University Card Request form, then schedule an appointment at the University DukeCard Office to have your DukeCard printed.
University Employees
Go to Upload Your Photo to submit your photo online. Schedule an appointment to visit the University DukeCard Office, then download and complete the University Card Request form. Bring the form (completed by you and your department) to the University DukeCard Office to obtain your card.
University Sponsored Guests
All sponsored guests (contractors, visitors, or volunteers) must be sponsored by a school or department through the OIT Sponsored Accounts Page. All sponsored guests must go to the office they are affiliated with to receive a DukeCard.
