What do I need in order to get my DukeCard?
   New employees should bring either an IR or a copy of the ID Request Form from the department in which they will be working. The forms should include any credentials (MD,PhD,RN,etc.) which need to be printed on the card as well as the employee's Duke Unique ID number.
    Note: Managers should include the complete fund code on the form so that we may bill the department correctly.  The cost is $5 for a regular card and $10 for a prox/HID card.

What should I do if my DukeCard is lost or stolen?
    Lost or stolen DukeCards must be reported to our office immediately. You can reach us 8-4 Mon-Fri at 684-2273 or 24 hours/day at 684-5800.  Your card can be deactivated instantly, blocking the unauthorized use of your accounts or access features.
    Replacement cards can be issued from the DukeCard Office at any time.
A new card can be produced in minutes and will have the same account and
access features as your original card.  There is a $10 fee for replacement of a lost or stolen card ($15 for prox/HID). This can be paid by the employee (cash or check only), or the department by IR or the ID Request Form.

How do I go about changing my name on my DukeCard?
    Employees who legally change their name can bring a marriage certificate or updated driver's license to our office and we will happily update their DukeCard. There is a $4 charge for this service.

How do I change my department or add credentials to my DukeCard?
    For transfers between departments employees need to bring an IR or ID Request Form filled out by the department to which they are transferring.  The employee should also bring their old ID card to our office or turn it in to the supervisor in the department they are leaving.

What should I do if my DukeCard gets damaged?
    If your card is damaged by normal wear and tear simply bring it by our office and we will replace it at no charge. However, if the card has been damaged by employee abuse (pins poked through it, dog ate it, etc.) there will be a $5 replacement fee ($10 for prox/HID card), as long as the old card is turned in.

How can I prevent my DukeCard from getting damaged?
    In order to protect you DukeCard from damage you should use the same care you would with a credit card.  In the hospital avoid going near the MRI with your badge on. It would be wise to keep your DukeCard away from any kind of strong magnet. Don't puncture, bend or fold your card. Don't accidentally put it in the washing machine. Also, you should not use your DukeCard as an ice scraper on those rare occasions when we have bad weather.

My DukeCard won't read when I swipe it, what should I do?
    There may be any number of reasons why your card isn't working. It is difficult to diagnose the problem over the phone and even harder to fix it that way. For that reason we recommend that you bring your card to our office if you feel that it is malfunctioning in any way. There is lots of testing equipment here and we can make sure that your card is working properly before you leave.

Can I have more than one DukeCard?
    One of the primary functions of the DukeCard is security.  In the interest of better controlling access to restricted areas and facilities, it is our policy that no person may have more than one active card at a time. If you have a replacement card made we deactivate any previous ones immediately to prevent any unauthorized use. This policy is geared toward the security concerns of all of us here at Duke and is not subject to exception.
